New Patient Referral

A referral is required for all new patients. Please contact your doctor and ask them to fax a referral order including reason for referral, office visit notes, imaging reports, insurance, and demographics to 203-624-4301. Referrals are evaluated to determine if the patient is an appropriate candidate for our treatments.
