A referral is required for all new patients. Please contact your doctor and ask them to fax a referral order to 203-624-4301.

*Medications & Addiction

Narcotics or opioids are potent medications related to morphine that are used to treat pain and its related symptoms. Narcotics have the potential for misuse and addiction and are thus, carefully controlled and are not usually the first route of pain management. No medications will be prescribed on the first visit. We follow strict state and federal guidelines regarding the use of opiate medications.

We are pleased you have chosen Advanced Diagnostic Pain Management Centers for your treatment. Your first visit usually takes about one to one and a half hours to complete and will include an evaluation by your pain doctor. No procedures are performed and no medications are prescribed during your first visit.*

​​Please bring the following to your first appointment:

  • Completed new patient forms. Arriving without completed forms may result in your appointment being rescheduled to another day.

  • Photo ID

  • Insurance card

  • Referral or authorization (if required). If your insurance policy requires a referral you must obtain one prior to your appointment time. This means being sure your primary doctor has called in a referral to your insurance company; otherwise your appointment will be rescheduled.

  • Cash, check or credit card for payment. If your policy requires a co-payment be prepared to pay this amount at the time of service. All self-pay balances are due at the time of service. If you have no coverage for pain service under your policy, please come to your appointment prepared to pay in full or call to make payment arrangements in advance.

  • Imaging film on a disc such as MRI, X-ray, CT Scan

  • Please have records relevant to your pain including previous pain management records faxed to: 203-624-4301.
